Formal DMCA takedown notices

If you're a copyright holder submitting a formal DMCA notice, include:

• A description of the copyrighted work you claim is infringed
• The exact URL(s) of the infringing material on this site
• Your contact information
• A statement that you have a good-faith belief the use is unauthorized
• A statement, under penalty of perjury, that the notice is accurate and you're authorized to act
• Your physical or electronic signature
